Full-time Registered Nurse (RN) – Medicare Health Coach position opening at Colville Community Health Center. This job position helps improve the overall health of the communities we serve by managing patient care. Job duties are as follows:

  • Function as a patient advocate to improve overall health outcomes and meet the patients wherever they are in their healthcare journey.
  • Coordinate care with various local health and social service organizations for ongoing care.
  • Determine Open Risk on managed health care patients and schedule patients to address this.
  • Educate patients on the importance of follow up visits and continuing medical care.
  • Facilitate patient care and document communications
  • Triage walk-in patients
  • Perform patient assessment, analysis, nursing diagnosis, planning, implementation and evaluation of nursing care and health teaching.
  • Define the history of present illness, update medications and collect a comprehensive medical, surgical and social history.
  • Perform mental health, chemical dependency and functional needs assessments.
  • View and interpret patient insurance eligibility
  • Oversee and delegate tasks to LPN and MA staff.
Valid Washington State RN License required. Bachelor of Science degree in Nursing from a four-year college or university preferred. Experience in a clinic setting as an RN preferred. Previous experience in triage and a community health clinic preferred. Excellent customer services skills and compassion for patients required. Competitive wages (DOE) and benefits. Partnerships

UW-RIDE :   The Regional Initiatives in Dental Education (RIDE) program is a specific educational track of the University of Washington School of Dentistry (UWSOD) that trains dentists to meet the needs of rural and under-served populations in the state and region.  The main goal of RIDE is to increase the number of graduates who will choose to practice within rural and under-served communities in Washington.  The RIDE curriculum includes a significant amount of training in rural and under-served areas to expand students’ familiarity and comfort in providing dental care for rural and under-served populations.  NEW Health Programs Association is a direct recipient of the RIDE program.

EWU :  At the NEW Health Program Association's Lake Spokane Clinic, we have the unique opportunity to have senior EWU dental hygiene students complete off-campus rotations with us. The students get to provide dental hygiene services to a variety of patients with direct supervision from our full-time hygienist.   It is an excellent opportunity for the students to have “real world” clinical experience as well as increasing the number of patients that we are able to provide service for.
