Colville Community Dental Clinic is located in the beautiful Colville valley. Our staff would like to welcome you and your family to our clinic. We appreciate your need to have access to quality and affordable dental care. We will do all we can to ensure that those needs are met. The dental clinic is certified to accept Medicaid (DSHS coupons) and is licensed by the State of Washington. We also offer sliding fee discounts that are based on family income and size.

Appointments:

• To schedule an appointment, please call 684-1440 or 1-800-948-3049 between the hours of
  8:15 a.m. and 5:00 p.m. Monday through Friday.
  
• If you cannot keep your scheduled appointment time, please call the clinic at least 24 hours in advance.
  
• If you arrive more than 15 minutes late for your appointment, you may not be seen that day.
  
• If you must cancel your appointment, we will try to reschedule you in as timely a manner as our schedule allows.
  

Sliding Fee:

We will need a copy of page one of your most recent federal income tax return (form 1040 or 1040A). If this is not available, or you did not file, we will require some other verification of level of income and number of family members. Copies of wage statements, DSHS coupons, unemployment / pay stubs, etc. may be substituted for members of household verification. A copy of your current Washington State Driver's License or current utility bill with your name and street address is also required. This information is used only for determining your eligibility for sliding fee discount and is held in strict confidence.
