What is a sliding discount?

How is a reduction in fee determined?

The Sliding Fee Discount is determined based on income level and the number of members in the household. Depending on the level of sliding fee qualified for, a percentage of the visit service charges will be discounted. The lowest nominal charge amount is $20.00 for a medical visit or $40.00 for a dental office visit.

How can I qualify for a sliding discount?

To qualify for a sliding fee discount the patients income must be at or below 200% of the federal poverty guidelines. Patients need to provide documentation of their income and the number of members in their household.
